Blepharoplasty is a very popular facial anti-aging procedure. It eliminates sagging skin and wrinkles around the eyes, resulting in a rested appearance. The impact of this procedure is clear, but can it actually change the shape of your eyes? This blog discusses exactly what blepharoplasty can and cannot do.

What Is Blepharoplasty?

Blepharoplasty, or eyelid surgery, is a facial cosmetic procedure that treats the upper, lower, or both eyelids for signs of aging.

Michael A. Persky, MD, is an experienced facial plastic surgeon in Encino, CA, who routinely performs upper and lower blepharoplasty to help patients achieve a more youthful, alert appearance. Below, we will discuss Dr. Persky’s approach to blepharoplasty, as well as take a look at some of his fantastic blepharoplasty before and after images.

What Does Lower Blepharoplasty Do?

Lower blepharoplasty treats what most patients consider to be under-eye “bags” or under-eye puffiness. If you have constant eye puffiness, even when you get plenty of sleep, you may be a candidate for lower blepharoplasty because what’s causing the bags under your eyes are, in fact, dislodged or displaced fat deposits.

During a lower blepharoplasty, Dr. Persky makes a small incision along the inside of each eyelid. Through this incision, he can tighten lax muscles and remove or reposition fat deposits to create a smoother result. He can also tighten the lower eyelid tissues to treat drooping eyelids.

The result of lower blepharoplasty is a more rested, youthful appearance.

What Does Upper Blepharoplasty Do?

Upper blepharoplasty is ideal for patients with sagging upper eyelid skin that droops over their eyes, fat deposits that cause puffiness, and excessive wrinkling.

The following images are of facial plastic surgeon Dr. Persky’s blepharoplasty patients, and you can see the smoothing effect the procedure creates. Through small incisions in the upper eyelid fold, Dr. Persky removes excess fat, muscle, and skin to open up the patient’s vision, smooth wrinkled, sagging skin, and create a more refreshed result.

Image on the right depicts him after surgery with a visible eyelid crease, showing more of his eye; he appears happier and more alert.

As you can see, these patients have a much more rested, youthful appearance, and the results are very impactful. But does this procedure change the shape of the eyes?

Blepharoplasty Does Not Change the Patient’s Eye Shape

In short: no, blepharoplasty does not change the patient’s eye shape. But what it can do is lift the upper eyelids to “open up” the eyes—or expose more of the actual eye rather than allow it to be shielded by hanging skin.

In addition to a larger appearance to the eyes, upper blepharoplasty has a positive aesthetic effect on the overall face, as the patients look happier, more rested, and more alert. By looking at the images of Dr. Persky’s patients, you can see that in the before images, the patients look tired, sad, or older than their age.

However, in the after images, they appear much more rejuvenated, youthful, and rested. Upper blepharoplasty is the biggest bang for the buck in appearing more rested. The effects of surgery cannot be denied: the removal of excess eyelid skin imparts a more alert and youthful appearance that impacts the patients’ eyes and overall countenance.
