30 year rhinoplasty results are wearing down: tip projection is lowering. What can be done?

I had a closed rhinoplasty 30 years ago to remove a hump and refine the tip. The operation was a success that it produced a satisfactory concave profile and refined tip. But over the years the shrink wrap effect has decreased the projection of the lower half of the nose (upper lateral cartilage to the tip), while the top half (nasal bone) is staying at the same height. Is it possible to re-achieve a concave profile, by shaving more on the bridge? Keep same tip, closed or open rhinoplasty?
A.Post rhinoplasty noses tend to shrink wrap over time. That is why it is essential that your initialrhinoplasty surgeon leave an adequate structural support of nasalbone and cartilage.You may either have a revision rhinoplasty or have a non-surgical filler injection to raise the tip of your nose. If you were my patient, I would try to use fillers first. Make sure to choose your injecting surgeon most carefully as previously operated upon noses have a much higher risk of complications. Should you decide upon the surgical route, once again choose your surgeon very carefully. Good luck and be well.
