Actual RealSelf Patient Comment: I am 62, and getting ready to do a lower facelift. One of the surgeons I consulted with recommended doing fat transfer at the same time, the other recommended doing fillers at a later date. I could definitely use something to fill in lost volume in my cheeks and under eye hollows. Which is the better option? Is fat as predictable as fillers would be? Other than the permanence, do they both yield similar results? Thank you!

A: As long as you are undergoing surgery, you might as well have fat used to restore volume. As with anything aesthetic, it is imperative that your surgeon is an expert as fat or fillers for that matter are not standard “commodities”, rather the results are extremely dependent on your physician. Fat is not as reliable as fillers as the amount that takes and remains permanent varies from 40 to 60%. The fat that does survive past the first 6 months will be permanent. Again, the most important aspect of your lower facelift will be choosing your surgeon most carefully. I strongly recommend Ari Hyman, MD for a natural beautiful result. Good luck and be well.
